Understanding Dental Implants

Although dental implants are nothing new to the dental field, there is still a bit of confusion as to their makeup. This is likely because dental implants come in a wide variety of shapes and sizes, each to accommodate specific needs.

For example, some implant systems are designed to support non-removable teeth while others may support teeth that are detachable for cleaning. Some implants support one or several teeth while others are designed for placement in minimum bone mass.

While dental implants have one of the highest of all implant-in-bone success rates (including that of knees and hip joints), having a successful outcome can greatly depend on having the proper implant selected for you. This can occur when you rely on an experienced dental specialist who is trained in the diagnosis and placement of all types.

To help clear up confusion about how implants work, it is important to understand the components. Below is an explanation of what dental implants are and how they work.

•  Dental Implants are actually replacements for tooth roots. The implanted portion is placed in your jaw bone where a tooth root was once held.

•  The ‘implant’ is not the tooth. It will help to hold the tooth (referred to as the ‘restoration’) but its primary function is to serve as the anchor for an attached tooth, or a replacement tooth root. By being placed in the jaw bone, just as a natural tooth root, the implant can support the attached tooth with a firm foundation for optimal stability.implant-components

•  The implant becomes anchored in the jaw bone through a process known as ‘osseo-integration.’ The word ‘osseo’ refers to bone, and the integration process is like a rope tied around a tree branch. In time, the branch grows around the rope. Osseo-integration is like this but occurs at a much more rapid pace.

•  Once the implant has become secured by the bone, a post is inserted into the hollow center of the implant. The replacement tooth (or teeth) is attached to this post. You new tooth looks, feels and functions like the natural tooth once positioned there.

• An implant is not always necessary for each missing tooth. In some instances, one implant can hold two or a bridge of teeth. Several strategically-placed implants can also support a full arch of teeth in many cases.

Regardless of the type of implant placed, it is important to remember that implants do occasionally fail. This can be due to clenching or grinding teeth. Smoking complicates and delays the healing process and can also contribute to implant failure. If infection sets in and reaches the implanted portion, the implant may need to be removed so the treatment can resolve the problem.

A major benefit of dental implants is their lifespan. Implants are designed to last your lifetime. Too, dental implants do not decay, will never need a root canal and do not rely on support from neighboring teeth. The best thing, however, is that dental implants restore your ability to eat with stability, chew comfortably, laugh with confidence and speak without worry.

Receded Gums – How They Occur & Why They Up Your Risk For Problems

The next time you look in the mirror, smile. Notice how each tooth is arched by gum tissue. This tissue seals off and protects the tooth root sections of teeth. When the gums pull away from the teeth, darker and sensitive portions of the root are exposed. This leaves tooth roots exposed and vulnerable.

While the darker segments of teeth detract from the appearance of a smile, they are also highly sensitive. Many people often first notice gum recession when getting a painful jolt when eating or drinking something hot or cold or even from the bristles of a tooth brush. Because gum recession occurs gradually, people don’t always notice it until they experience pain.

Special toothpastes may help with sensitivity, however, it’s best to determine what has led to gum recession so further recession doesn’t occur. Causes for receded gums include:
• Periodontal (gum) disease: When bacteria accumulates on teeth at the gum line, the gum tissues weaken and lose their grip on teeth. As gum disease progresses, the tissues and supporting bone are overwhelmed by oral bacteria.
• Rigorous brushing: Using a tooth brush with hard bristles or being over-zealous when brushing can wear down enamel as well as gum tissue. Also, using abrasive substances to brush (such as baking soda) are too gritty for teeth and can wear down gum tissues.
• Poor oral hygiene: Without thorough and regular brushing and flossing, oral bacteria accumulate and form a sticky film known as plaque. When plaque is not removed, it can harden into calculus, which is a cement-like colony of oral bacteria. Calculus (or tartar) can only be removed by a dental professional or it will continue to feed on gum tissues.
• Smoking: When saliva flow is insufficient to effectively wash bacteria from the mouth. The chemicals in tobacco are very drying to oral tissues, providing an ideal environment for the formation of plaque and calculus. Both are masses of oral bacteria that attacks gum tissue and contributes to recession.
• Grinding & clenching teeth: Clenching or grinding teeth during sleep can place such force on teeth that they tilt out of their positions. Eventually, the gums pull away from some teeth, exposing tender tooth root sections.
• Hormonal changes: Pregnancy, menopause and puberty can cause changes in hormone levels. These hormonal fluctuations can cause gums to feel tender and be more vulnerable to recession.
• Crowded or crooked teeth: When teeth are not properly aligned, they endure added force to bite and chew. This can also lead to strain on the TMJ (jaw joints), gums and bone structures that support tooth roots. This can eventually result in gum recession.

When darker portions of a tooth are visible, it detracts from a smile and leaves the tooth susceptible to decay or gum disease. For the comfort, appearance and health of the tooth, repairing the problem before costly damage occurs is the best move.

You Can Help Your Dental Implant’s Success

Over the years, the design and placement process of Dental Implants have been so perfected that they’ve come to hold one of the highest success rates of all in-bone implant types – nearly 97%.

One of the reasons that Dental Implants are such a wise investment is that they are designed to last your lifetime. However, there is a potential for failure. Like any procedure that involves an implant in human bone (including hip, knee, etc.), there is an element of risk.

Having an implant removed due to infection or malfunction is difficult for the patient. Although it’s not a frequent occurrence, removing an implant means the patient has lost their investment and must endure additional procedures and expenses for repair and replacement. How can you help to minimize this risk?

One of the most important factors in any successful medical or dental procedure begins with the doctor who coordinates your care. Your doctor should never be selected based on the lowest fee. The doctor chosen should have extensive training and experience in the diagnosis and placement of all types of Dental Implant systems. This enables him or her to select an implant that is appropriate for your individual needs.

In selecting the doctor who will oversee your treatment, you should also look for advanced skills in the placement of your implants. Successful placement relies on the ability to assess for adequate bone mass to support an implant without interfering with adjacent structures. The ability to select the proper implant type for available bone depth is equally important.

Without proper placement in sufficient bone, an implant risks coming in contact with a nerve that extends horizontally through the lower jaw. While the doctor involved in your treatment is important, much of the risk falls into the patient’s hands after the placement process.

As a patient, helping to have a successful outcome begins as soon as your implants are placed. First, closely follow your post-placement instructions. For a few days following placement, most patients are advised to eat only cool, soft foods. This helps to minimize swelling and bleeding, which helps gum tissues to more quickly seal incision sites. This can lower your risk for infection.

Once home, other factors can also place your implants at risk. For example, smokers have a higher risk for implant failure. Because the chemicals in cigarette smoke are very drying to oral tissues, the healing process takes longer. Delayed healing creates a greater risk for infection to occur.

An element of risk that may surprise you is grinding or clenching teeth during sleep, which is typically a symptom of a misaligned bite. Some clenching is so intense the force can be likened to that used to crack a walnut. A sign of night-time clenching or grinding may be worn, chipped or broken teeth. However, even without signs, if you suspect you grind or clench, mention this to your implant dentist prior to treatment. This way, proactive measures can be taken to resolve the problem before complications result.

Most important of all is the patient’s commitment to maintaining good oral hygiene. Although Dental Implants themselves do not experience decay, the gum tissues and bone supporting the implants are as susceptible to oral bacteria as before. When oral bacteria infection (gum disease) penetrates to the implant site positions, the only way to treat the infection may require removing the implant.

Having Dental Implants will require you to be highly committed to your oral hygiene at home. Additionally, your dental check-ups will likely be scheduled for every four months than twice a year. During these visits, a hygienist will remove accumulated oral bacteria to reduce risk to your Dental Implants. The condition of your gums will also be assessed.

A Missing Tooth Disrupts Positions Of Other Teeth

Being in a periodontal specialty, a particular part of my expertise involves the diagnosis and placement of Dental Implants. While periodontal (gum) disease is the nation’s leading cause of adult tooth loss, there is another contributing factor that many people are unaware.

The position of each tooth has unique importance to neighboring tooth. Primarily, one tooth helps to hold adjacent teeth in place. This means that when one tooth is lost and not replaced, the teeth on both sides are also left without a bolstering neighbor. This gap enables neighboring teeth to move out of their proper positions.

Because an open space allows a tooth above or below to grow longer or the teeth on each side to tilt, a long list of problems are set into motion. The teeth that grow longer or tilt typically cause chips, cracks or fractures to other teeth since the bite is now out of alignment. When a tooth fractures below the gum line, it will require removal. This results in a whole net set of expenses and problems.

When a tooth is missing, it also leads to the potential shifting of other teeth. This shifting can result in crowded, crooked teeth that are more difficult to keep clean. Imagine how much more challenging it is to sweep and mop the floor in corners than it is along straight surfaces. Your tooth brush has far more challenges trying to get into tight angles.

Bite misalignment can even lead to worn teeth since it can lead to night-time clenching and grinding. This is typically due to strain on the TMJ (jaw joints) from the now-misaligned bite. Clenching or grinding during sleep can contribute to frequent headaches, migraines, sore jaw joints, facial and neck muscle pain, ear ringing and dizziness.

The open space from the missing tooth leads to yet another ordeal. Without natural tooth roots in the jaw, the bone that once held the root begins to shrink. As the bone in this area declines in mass, bordering tooth roots are now vulnerable to a shrinking foundation of bone. It is a fact that a missing tooth leaves an adjacent tooth most likely to be lost next.

When you consider all these issues, it’s no surprise that the result is a number of dental repairs and costs. The easiest way to avoid problems and associated expenses when a tooth is lost is to replace the tooth with a Dental Implant.

Dental Implants recreate the presence of tooth roots in the jaw bone. This helps to prevent bone loss from a declining jaw that occurs when tooth roots are no longer present. The replaced tooth also helps surrounding teeth to hold their proper positions, greatly reducing the potential for chips, cracks and the barrage of problems associated with bite misalignment.

Although Dental Implants are initially more expensive than most other tooth replacement options, the investment is a one-time expense. Dental Implants do not decay and are designed to last your lifetime. They also have one of the highest success rates of all implant-in-bone treatments.

And, unlike crown-&-bridge combinations, an implant will halt further bone loss. It will never need adjacent natural teeth to be crowned to support replacement teeth. Dental Implants are held in the jaw bone, just as natural tooth roots, and do not need other teeth for support. Overall, they are an ideal investment for replacing one tooth, several teeth or a full arch.
